TRACY KULIK, M.S.P.H.
President and Co-Founder
 

Ms. Kulik has 30 years of healthcare industry experience in hospital administration and health care consulting.  She is expert in the funding, planning, delivery and evaluation of safety net services, with substantial experience in public sector risk product development and quality management. 

At Collaborative, Tracy manages complex projects including: evaluation of market demand based on geographic, population-based demographic and epidemiologic factors; service needs analysis; and identification of gaps and barriers for vulnerable populations.  Her clients include states and municipalities; hospital systems, individual hospitals, payers and professional societies.

Earlier positions in hospital administration and consulting include: Senior Manager, Managed Care/Marketing for Kurt Salmon Associates (previously Hamilton.KSA); SVP Quality Improvement for Franciscan Health System in Dayton, OH; Associate Executive Director Managed Care/Quality, Columbus Children’s Hospital in Columbus, OH; and Corporate Director of Managed Care for Alliant Health System (now Norton Healthcare) in Louisville, KY.

Tracy is a Certified Professional in Health Quality (CPHQ), is certified in the American College of Utilization Review Professionals (ACURP), and is a fellow in the American College of Healthcare Executives (FACHE). 

Representative Accomplishments

  • Created and implemented the quality improvement process for Alliant Health System (now Norton Healthcare in Louisville, Kentucky) which was the inaugural winner of the Witt Healthcare Forum Award for Total Quality Management.
  • For a large Northeastern Ryan White program, increased compliance with Standards of Care from 70%  to 92% over three years with improvement of Primary Medical Care from 88% to 100%
  • For Midwestern TGA, reduced Out of Care population from an estimated 28% to less than 15% by enabling the tracking of Unmet Need individuals
  • Helped three Ryan White programs in initial year of grants assumption to improve grant scores from low 80’s to mid-90’s
  • Created a grants infrastructure and funding base for health disparities for a large municipality addressing chronic disease; obtained $2M annually in grant revenue.
  • Developed reimbursement, service definitions and enabling legislation for six of the eight initial Medicare+CHOICE pilot programs (Medicare risk).
  • Drafted enabling legislation for State Children’s Health Insurance Programs (SCHIP) for the states of Arkansas, Georgia and Kentucky.

Education
Tracy earned her Bachelor Degree in Premedical Science from Canisius College in Buffalo, New York and her Master of Science in Public Health from the University of Massachusetts in Amherst, Massachusetts. .

JEFF S. DANIEL
Chief Executive Officer & Co-Founder
 

Mr. Daniel has seven years experience in the HIV/AIDS field, primarily focused on Ryan White entities. He served on the Austin, TX TGA (Ryan White Part A) Planning Council for four years as Chair of the Needs Assessment and Priority Setting committee (1998-2002) and as Vice Chair of the Planning Council (2000-2002).  Jeff has had substantial success reaching out-of-care populations and collecting public health data for this underserved group.

In addition to his role in leading needs assessments, in care studies and Priority Setting and Resource allocation processes, he is responsible for all financial, administrative, and contract matters for the Firm.

Earlier in his career, Jeff was director of recruiting for Trilogy Software, where he enabled the company to grow from 50 to 1500 employees in five years.  He was the founder of College Hire, a specialty staffing firm for clients such as Goldman Sachs, Amazon.com and Siebel Systems.

Representative Accomplishments

  • Conducted first ever out of care needs assessments for the Sacramento TGA, Phoenix EMA, Denver TGA, San Antonio TGA, New Haven TGA, Nassau-Suffolk TGA and Norfolk TGA
  • Led the San Antonio EMA Needs Assessment in reporting over 100 out-of-care individuals, enabling  their Planning Council to better set priorities and target resources
  • Developed the 2008 Nassau Suffolk Priority Setting and Resource Allocation methodology which was cited as a strengthen in the TGA’s 2008 HRSA Part A grant application
  • Supported the Phoenix EMA grantee in authoring the EMA’s 2007 and 2008 HRSA Part A grant applications resulting in a 4% ($262,146)  and 7% ($531,468)  funding increase respectively

Education

Jeff holds a Masters of International Journalism and a Bachelor of Arts in Journalism and Marketing from Baylor University in Waco, Texas.  Jeff speaks conversational Spanish.

EDIE PAYETTE
Director of Special Studies
 

Edie brings 25+ years of sales and operations experience to her position.  She has worked extensively in bilingual education and curriculum development, including working closely with special education programs.  Her clients have included school districts in El Paso, San Antonio and the Rio Grande Valley Texas.   In her role at Collaborative she oversees billing, administers needs assessment surveys, and manages Collaborative’s San Antonio office.

Representative Accomplishments

  • Managed a territory consisting of 76 counties in South and West Texas, ensuring client satisfaction in order processing and product delivery
  • Qualified for the Company’s Platinum Panel for exceeding sales expectations in 2004, 2005 and 2006
  • Served on national educational panels on English language acquisition
  • Liaised with editorial staff to develop appropriate curriculum for non-English speaking students

Education
Edie attended Texas A&M University in Kingsville, Texas majoring in English with a minor in Journalism.
   

LISA DANIEL
Vice President of Finance
 

Lisa has a great variety of work experiences that she brings to Collaborative Research. She taught school for 6 years in the public school system and has also spent the past 17 years working in the fitness industry. In her position at Collaborative, she oversees billing and banking transactions as well as contract employee information. She also performs data entry and analysis for needs assessment surveys.

Representative Accomplishments

  • Received Exceeds Expectations evaluation ratings all 6 years and served as campus Math Facilitator while teaching in the public school system.
  • American Council on Exercise nationally certified fitness instructor since 1996.

Education
Lisa holds a Master of Science in Educational Administration from Texas A&M-Kingsville and a Bachelor of Business Administration from The University of Texas at Austin. She also has her teaching certification with a specialization in Math from The University of Texas at San Antonio.
   

JENICE CONTRERAS, E.M.B.A. candidate
Senior Manager
 

Ms. Contreras has extensive healthcare industry experience in healthcare operations, research and grants. She is proficient in the complex process of integrating national and regional thought leadership and need justification into a cohesive grant submission. She applies the tenets of grant award into a consistently well-managed operation with written validation. She is bilingual and bicultural, and is extensively involved in minority leadership efforts in her community.

At Collaborative, Jenice manages health access projects and federal grant submissions. She contributes to quality improvement efforts for the firm, with prior experience managing a significant Ryan White Part C grant at Care Alliance and a SPNS (Special Projects of National Significance) grant. Enhancing Linkages to HIV Primary Care & Services in Jail Settings was one of 20 SPNS grants awarded nationally, competing with over 300 submissions. Project officers from Abt Associates and Emory University Rollins School of Public Health rank her project management in the top 3 among the 20 awardees over the four years since grant award.

Representative Accomplishments

  • Director of Development and Grants for Care Alliance Health Center, the only HRSA Bureau of Primary Health Care funded non-profit Federally Qualified Health Center in Cleveland, Ohio that focuses on the delivery of primary health care and support services to homeless persons and residents of public housing.
  • Designed comprehensive HIV Services department at Care Alliance Health Center through the support of Ryan White Part C-Early Intervention Services program, among other Federal and local funding sources.
  • Fellow, Ladder to Leadership: Developing the Next Generation of Community Health Leaders, a collaborative initiative of the Robert Wood Johnson Foundation (RWJF) and the Center for Creative Leadership.
  • Researcher in the Department of Epidemiology and Biostatistics of the School of Medicine at the Center for Minority Public Health of Case Western Reserve University. During her four years at CASE, Ms. Contreras developed marketing strategies for recruiting research participants and built relationships with community organizations.
  • Americorps National Service Volunteer in 1997 with service at the West Side Ecumenical Ministry, a local non-profit organization.
  • Youth Services Coordinator for the Americorps program, responsible for supervision of the National Service Volunteers as well as daily operations of the program.
  • Family Service Worker at the Head Start Program of the West Side Ecumenical Ministry then promoted to coordinate the program's marketing and recruitment.

Education
Jenice earned her Bachelor's of Arts degree from Capital University. She is currently attending The Ohio State University, Fisher College of Business in the Executive Master of Business program. She is on the Board of Trustees for the North East Ohio Hispanic Chamber of Commerce and was appointed by Mayor Frank Jackson to serve on the City of Cleveland, Community Relations Board. She is a member of the American Fundraisers Association and the Young Latino Net.
   

RHONDA STEWART, BS
Senior Manager
 

Ms. Stewart has seven years experience in the HIV/AIDS field, primarily focused on Ryan White Ambulatory Outpatient Medical Providers. She served on the San Antonio, TX Transitional Grant Area (Ryan White Part A) Planning Council for 4 years as Chair of the Needs Assessment committee (2004-2008) and as Co-Chair of the Planning Council (2007-2009). Rhonda has extensive knowledge of the Ryan White Modernization Treatment Act specifically with Quality Management, Outcome Measures, and Client Level Data in the medical community. She served as Director of Specialty Clinics for 5 years as the Part C Representative for the SA TGA.

Representative Accomplishments

  • Presented at Ryan White All Grantees Conference in 2005 as a Part C Representative regarding Client Level Data and bringing Out of Care into Care.
  • Nominated by Department of State Health Services in Texas for participation in the Statewide Collaborative in 2009.
  • Directed multiple medical clinics in the public health community in San Antonio TGA for CentroMed, to include the Federal Healthcare for the Homeless grant clinics.
  • Collaborative Research, LLC Senior Manager for the 2011Standards of Care revision with San Antonio TGA/HSDA Planning Council
